This kit comes equipped with tape lifts, slides, plastic slide containers, Chain of Custody form, ID labels to ensure proper sample identification & pre-addressed Priority Mailer. Meets IESO standards for sampling.
- Remove the paper backing from the tape lift kit.
- Taking care to avoid touching the adhesive area on the tape lift to be used for the sample, apply the defined area of the tape over the surface you wish to sample.

Lift tape off surface and apply to the enclosed microscope slide.
- Place the slide into the protective case.

Fill out the laboratory chain of custody. The laboratory test code is S001T
Place protective case containing tape lift sample and chain of custody in a shipping box and send by U.S. Mail, FedEx or other carrier to our lab. <><>
Lab results sent in 5-7 days.
